INSTRUCTIONS:
Read the entire form before you begin to answer so you may organize your responses.
No fewer than two nominations per school will be considered.
1. Student Eligibility is based on "passing" grades provided they reflect the best ability of the student.
2. Evaluators make their selections based solely on the responses and supporting detail provided.
3. Support for the nomination may be provided in the form of letters and testimonials, which support the nominee's ability to demonstrate citizenship. Please be very specific.
4. Nomination forms must be endorsed by at least two interested citizens. For example, teacher, employers, parents, community leaders.
5. This nomination form is a portfolio where additional pages may be provided.
6. Please do not put information into a binder.
7. Please ensure all references to the student’s name are spelled correctly.
8. Complete each section of this form in legible script or print.
9. PLEASE, Do not include any information that may prove embarrassing to the student, as it may be inadvertently be disclosed in the public presentation at the awards ceremony. Remember this information is public. 10. DO NOT include confidential information.
11. MATERIALS WILL BE RETURNED.
